Un homme nommé Shouichi Tsugami a perdu ses souvenirs. Il ne sait pas qui il est, d'où il vient, ou comment il est venu sur ses circonstances particulières. Tsugami, apparemment sans raison, se transforme en un puissant surhumain, Kamen Rider Agito , chaque fois en présence des êtres désignés par la police comme "l' Inconnu ", une race de monstres puissants qui ont causé des meurtres autour de Tokyo, ciblant certaines personnes comme leur proie.
